What insurances do you accept?

Revelation Spine Center contracts with most major health insurance carriers. This list is updated periodically and is subject to change without notice. If you do not see your insurance listed please give us a call to verify. You can also contact your insurance company to better understand how your policy works and any financial liability that you may have, prior to receiving care.

How can I minimize my wait time?

By downloading the Healow app and checking in online. https://healow.com/app.html

You can also use Healow to schedule appointments, receive text reminders, check-in online, access your records, and pay your bill online.

New Patients

If you do not check in ahead of time; you will need to arrive 15 minutes early to complete the check-in process.

New patients, please bring with you:

• Photo ID
• Insurance Card
• Form of payment
• List of medications you are currently taking
• Name and address of the pharmacy you prefer to use for any prescriptions

What if I don't have insurance?

If you do not have insurance or we are not contracted with your insurance company we offer fair self pay rates.
